All families are referred to Project 12-Ways through their local DCFS field office or other case management agency (such as Caritas Family Solutions, Lutheran Social Services).

To be eligible for services, families must meet the guidelines specified under Title XX rules and must reside in on of the following counties:

Franklin, Gallatin, Hamilton, Jackson, Jefferson, Perry, Pulaski, Randolph, Saline, Union, Wayne, White, Williamson


Referral Process

Referral Received

After a referral is received from the family's case worker, the project staff will screen the referral for appropriateness. Letters are sent to the referring agency to notify the case worker of acceptance or rejection of a referral. 

Referral Accepted

After a referral has been accepted for services, the project staff meet with the family weekly in their home. Staff work closely with the referring agency to report on the each family’s progress with services.
